USF COVID-19 Vaccine Program
USF Health has exhausted its available supply of COVID-19 vaccine and has not yet received additional vaccine from its hospital partners or the state. As a result, the university has suspended further invitations for COVID-19 vaccine first shot appointments. While USF is now out of vaccines, it is important that the university know who needs to be vaccinated for the development of a queue when more doses arrive.

USF has developed two surveys to gauge interest in the vaccine and assist in scheduling appointments once additional doses of the vaccine are made available. Those interested can still submit a survey and will be added to the queue for future appointment scheduling.
  • If you are age 65 and older, please complete this survey.
  • If you are under 65 years of age, you are not currently eligible to receive the vaccine. However, you should still complete this survey.
Filling out the survey does not prevent you from seeking vaccinations from your county or any other venue. If you can get it locally just inform the university. The survey will take just a few minutes to complete!

Any vaccine related questions should be directed to vaccine-info@usf.edu.
